Join a dedicated team where you can make a significant impact by providing psychosocial support and collaborating with various stakeholders to enhance patient well-being.

Responsibilities:

  • Conduct casework and deliver psychosocial support to patients, caregivers, and families.
  • Review intervention plans and collaborate with all relevant stakeholders, including external agencies.
  • Manage patient admissions and perform assessments for financial assistance.
  • Plan, organise, and oversee social programmes tailored for patients.
  • Maintain and update patient records and billing information.
  • Lead and facilitate group work and programmes designed for targeted client groups.
  • Promote services to client groups and advise stakeholders on relevant programmes and services.

Requirements:

  • Degree in social work with accreditation.
  • Minimum of 2 to 3 years of relevant experience.
  • Strong dedication to improving patients’ lives through holistic social care.
  • Excellent teamwork abilities, along with effective written communication and interpersonal skills.
