At Minmed, we're making quality healthcare more accessible through innovation and technology. With up to 30 GP clinics across Singapore, join us in making a difference.

Our organisation supports public health institutions such as SGH, CGH, KKH, and NUHS in delivering Mobile Inpatient Care @ Home services. We are also launching a Senior Shared Stay-In Care Service to support seniors living in a shared care environment.
In this role, you will coordinate nurse home visits, plan and optimise daily routes, roster and deploy nurses, communicate assignments to locum nurses, liaise with hospitals and internal teams, and help resolve day-to-day scheduling challenges. You will also support client onboarding, contracts, billing, feedback management, and care coordination for seniors in the shared stay-in care service.

Roles and Responsibilities:

  • Plan and optimise daily routes for nurse home visits.
  • Liaise with hospitals and internal teams on confirmed patient visits and scheduling updates.
  • Oversee and allocate tasks to a diverse team of locum nurses, ensuring effective communication and accountability.
  • Track and manage part-time staff worklogs.
  • Resolve day-to-day scheduling and operational issues.
  • Maintain clear and timely communication with internal and external stakeholders.
  • Support onboarding of new clients and senior residents.
  • Coordinate admission arrangements for seniors entering the shared care facility.
  • Liaise with clients, family members, caregivers, care staff, and internal teams.
  • Assist with day-to-day care coordination for seniors living in the facility.
  • Manage, document, and follow up on client feedback, concerns, and service requests.
  • Support billing coordination, payment follow-up, and administrative records.

Requirements:

  • Diploma or degree in healthcare, business administration, operations, or related field.
  • Minimum 2 years of relevant experience in healthcare, eldercare, home care, or logistics coordination preferred.
  • Strong organisational and communication skills.
  • Proficient in using scheduling software and healthcare management systems to track appointments and coordinate care.
  • Excellent problem-solving skills with a strong ability to adapt to changing situations and client needs.

Pay: $3,200.00 - $3,700.00 per month
